Form W-9 is use to confirm an individual's Taxpayer Identification number (TIN), address, and name. It is one of the most commonly used IRS Forms. If you are a self-employed individual engaged with a company or business, and are expecting an income in return. Then, in such a case, the company/business will ask you to fill out Form W-9.

You will complete the Form W-9 and send it back to the company. With the information provided in the form, the company will prepare a 1099 form. Form 1099 will report the payment the company made to you by the end of the year.
